I usually embroider my name and who the quilt is to, along with the date on a back corner of the quilt.
I make labels for all my quilts. I used to use waterproof pens and actually sign, but found that my signature always looked like I was somewhere in second grade or so -- because the pens really don't want to write smoothly on fabric. Now I embroider them, using my machine. I include the month and year, my name, who it is to, if anyone, and generally a small embroidery design that reflects what's on the front. For quilts that aren't going out of the house, I embroider through from the front, with my initials and the date inside a heart - hiding the design somewhere in the border.
I print my labels on treated fabric with a label I've designed on the computer. I try to make the label design in the theme of the quilt. My pony express quilt had a label that looked like an old postcard. My spiderweb quilt had a label that was framed by spiderwebs.
